  • Am I eligible?
    You must be a US citizen or Green Card holder, actively at work with a minimum earnings based on your state of residence. Certain medical preconditions may make you ineligible, for example, if you have suffered a stroke, heart attack or had cancer in the last five years. You will be asked to complete a health questionnaire as part of the application process to determine eligibility.
  • Which states is Sick Pay Plus available?
    All states excluding Alaska and Washington.
  • Will I need a medical exam?
    No, you do need a medical exam. You will be asked to complete a health questionnaire as part of the application process to determine eligibility.
  • How many benefit types are there?
    You can choose from 3 monthly benefits: $800, $1000, $1500. *Subject to financial eligibility.
  • When does my coverage start?
    Benefits start the first day you are too ill or injured to work and are under a doctor’s care for a covered illness or injury.
  • What is the Benefit Period?
    The duration of your benefit period for any individual claim depends upon the severity of your illness or injury- either 2 months, 4 months, or up to 6 months. Minor injuries or illnesses, such as sprains, strains, flu and pneumonia up to two months. Serious injuries or illnesses, such as hospitalization, in or outpatient surgery, C-section, or results in a fracture up to four months. Major injuries or illnesses, such as heart attacks, cancer, strokes, paralyses and hip fractures up to six months.
  • Is there an offset if I have other disability or sick pay related policies?
    No, there is no offset.
  • Where can I manage my policy?
    Through the Combined Insurance Claims Portal here. You’ll be able to file and track claims, view your policy, download your documents, make secure payments, update your details and keep your account secure.
  • Can I change my coverage?
    Yes, call 1-800-544-9382 to discuss the change directly with Chubb, who will be able to help you make the change or provide the necessary forms, and answer any questions you might have. You can also take care of certain coverage changes yourself in the Self-Service Portal.
  • How much does it cost?
    Cost is determined by the amount of coverage you choose, discounts are available for applicants with a healthy BMI and if a non-smoker.
  • How do I pay the premium?
    You can pay by credit card or by bank debit. You are prompted to pay during the application process.
  • How do I make a claim?
    You can access the Combined Insurance Claims Portal here. You can read this document below explaining how to navigate the portal. .
  • How do I qualify for a claim?
    You must upload a doctor’s note to the Combined Insurance Portal.
  • How soon will I get paid?
    Approximately 3 weeks from the date of claim submission, if all required forms, documentation and information are provided in a timely manner and the claim is for a covered illness or injury and is approved. If a check is being sent, it will be received in approximately days from the claim approval date, subject to the USPS system. If an e-payment is being made, you will receive an email to create/register for an account. Payment will be completed within three business days.
  • How often do I receive my benefit?
    Benefits are prorated daily and paid out on a monthly basis based upon 30 days per month.
